Patient Complaint
The patient presented with restricted mouth opening (approximately two-finger breadth), causing difficulty in chewing, speaking, and maintaining oral hygiene. The condition was associated with discomfort in the lower back tooth region.
Clinical Findings
On examination, trismus (limited mouth opening) was observed. The patient was unable to open the mouth beyond two fingers, indicating muscular restriction and possible underlying pathology.
Radiographic Evaluation
An orthopantomogram (OPG) was advised for further assessment. The radiographic findings revealed a horizontally impacted mandibular third molar (wisdom tooth) embedded within the bone.
This impacted tooth was identified as the primary cause of inflammation and restricted mouth opening.
Diagnosis
Trismus associated with horizontally impacted mandibular third molar
Treatment Plan
Surgical extraction of the impacted wisdom tooth under local anesthesia.
Procedure Performed
Administration of local anesthesia
Reflection of mucoperiosteal flap
Removal of surrounding bone to expose the impacted tooth
Sectioning of the tooth (if required)
Atraumatic extraction of the tooth
Irrigation of the surgical site
Suturing of the wound
Outcome
Immediately after the procedure, a significant improvement in mouth opening was observed.
Pre-operative mouth opening: 2-finger breadth
Post-operative mouth opening: 3-finger breadth
The patient experienced instant relief from restriction and improved jaw mobility.
