15-Year-Old Male with 4 cm Maxillary Radicular Cyst (Central to Premolar Region)

π¬ What is a Radicular Cyst?
A radicular cyst (periapical cyst) is the most common inflammatory odontogenic cyst associated with a non-vital tooth. It develops due to chronic infection at the tooth root apex and gradually expands, causing bone loss and swelling in the jaw.

Patient: 15-year-old male
Complaint: Painless swelling in upper front jaw
Duration: 2β3 months (gradually increasing)
Region involved: Central incisor to premolar area
Tooth vitality: Multiple non-vital teeth (confirmed via pulp testing)
πΈ Clinical & Radiographic Findings
Clinical Exam:
Firm, non-tender swelling in anterior maxillary vestibule
Mild buccal cortical plate expansion
Discolored central incisor
No pus discharge or sinus opening
Radiographic Findings (Insert X-ray or CBCT):
Well-defined 4 cm radiolucent lesion extending from central incisor to premolar
Root resorption noted in adjacent teeth

The patient underwent surgical enucleation of the cyst under local anesthesia with apicoectomy of involved teeth.
Surgical Steps:

2οΈβ£ Complete removal of cystic lining
3οΈβ£ Apicoectomy of non-vital teeth
4οΈβ£ Irrigation with saline & betadine
5οΈβ£ Flap closure using 3-0 silk sutures
